HVA Registration Page

HVA accepts new players throughout the season. In order to register with us, its important that you contact the club. For contact information, click the "Contact us" link from the homepage. Next simply follow the instructions below. Registration is as easy as 1,2,3,4....

STEP ONE:   FEES

All forms and agreements must be completed in order for a player to be registered. A registration fee of $150 should be mailed with the (3) completed forms or brought to the parents meeting.  

STEP TWO:   ON-LINE INFO

The following are required forms and should be filled out in their entirety and signed by both the players and the parents and mailed to 826 Wavecrest Lane, Houston, Texas 77062.

STEP THREE:   FORMS

1. Print the following 3 forms and fill out according to the included directions:
   
   You'll need Acrobat Reader loaded on your computer to download and print these forms. Its free, simply click this yellow box. When you've installed it. Come back to our website, click register, and start right below the yellow box. Its just that simple.
   

• USA Volleyball Player registration form (Fill out SECTION I "Personal Info" Block only, then sign parent and player blocks in both SECTION I and SECTION IV. Disregard mention of fees noted at the bottom of Page 1 of this form. HVA makes the payment on your behalf.)

• USA Volleyball Youth medical form

• Hold Harmless Agreement   Must be Notarized
  

The (3) Forms must be mailed in to HVA or brought to the parents mtg.

STEP FOUR:   BIRTH CERTIFICATE COPY

1. Copy of the Player's Birth Certificate.

If you've completed the steps mentioned in the four areas above. You're Finished! Congratulations! You will receive a confirmation email at both the parents and the player's email address soon. Stay Tuned.
