USER ID Required
User access is very easy. You will be prompted for a USERID. The USERID is the first initial of the player followed by the last name of the player. For Example John Smith, USERID=jsmith 

Password Required
You will be further be prompted for a password. The password is the last 4 digits of the PLAYER'S social security number. There are some cases where the player does not have a social security number or has not reported that to HVA. In such cases, the player should enter his birth date numerically. For example, May 7, 1985 should be entered as 5785(no dashes or zeros should be used).
